Chronic Care Management

Catalina Island Medical Center offers Chronic Care Management (CCM) services to our patients for patients 65 years and older and patients who have Medicare coverage regardless of age who have two or more chronic conditions that are expected to last at least 12 months that increases the risk of death, the illness getting worse or permanently affecting their ability to do their normal activities. The goal is to keep patients as healthy as possible, keep patients out of the hospital, minimize costs as much as possible and continue to provide patients with the best care.

These services help manage the health of CCM patients between their clinic visits. Your CIMC Healthcare provider will work with you to coordinate your CCM plan with our Chronic Care Management provider, who will connect monthly with you to ensure your healthcare needs are being addressed.

Chronic Care Management services include:

  • a personalized care plan that will be created where the patient and provider work together to make goals that will improve their health
  • 24/7 access to care with help of a Nurse Advice Line when the clinic is closed
  • help with managing medications
  • help with coordinating care with other providers.

There may be a co-pay of $8.50/month for the service depending on the patient’s insurance.
