Please present your insurance card for all hospital and clinic services.
We, like any other medical facility, need to request your insurance card and ID along with updated contact information as it is crucial to keep your medical records, personal information, and insurance information up to date. Presenting a current and valid insurance card will decrease the risk of denial of payment.
If you have a co-pay for clinic, laboratory, x-ray, or emergency services, please be prepared to pay it.
